Referral requirements, clinical information and submission details for general practitioners and independent medical specialists.
For cosmetic surgery consultations, referrals are accepted from the patient’s usual GP or, where this is not possible, another GP or an independent medical specialist who does not provide cosmetic surgery or non-surgical cosmetic procedures.
Please arrange the referral before the patient’s first cosmetic surgery consultation with Dr Nara.
The referral should provide information relevant to assessment and perioperative care. Dr Nara will undertake an independent assessment of the patient’s indications and suitability for surgery.
In accordance with the Medical Board of Australia guidelines, the referral should follow a real-time consultation with the patient. A referral is not required for non-surgical cosmetic procedures.
Please use your practice’s usual referral template and include the following information where relevant. There is no separate RevAesthetic form to complete.
Patient name, date of birth and contact details
Reason for referral, including the procedure or body area where known
Relevant medical and surgical history, comorbidities and previous procedures
Current medications, allergies and relevant investigation results
Relevant mental health history or current care, where known
Referring practitioner’s name, practice, provider number and contact details
Please confirm the patient has consented to the referral and include only information relevant to their care.
Please send referrals by email or fax. Secure messaging through HealthLink or Medical Objects is not currently available.

Do not submit referrals or clinical records through the general patient enquiry form. If your practice policy requires a secure messaging provider, please contact reception to discuss the available options.
Our rooms will review the referral and contact your practice if further clinical or contact information is required.
The patient can then arrange a consultation with Dr Nara by contacting the rooms.
Clinical correspondence will be sent to the referring practitioner as appropriate.
Dr Nara consults in Melbourne, Tasmania and Adelaide for breast, body, face and intimate cosmetic surgery. Procedure information is available in the surgical procedures directory.
The rooms do not provide acute or emergency assessment. Please direct urgent concerns through the appropriate emergency pathway.
